Mercy Hospital St. Louis Critical Care Medicine Fellowship is seeking a PRN Clinical Pharmacist to optimize medication therapy in a fast-paced hospital setting.

You will collaborate with an interdisciplinary team, ensure safe dosing, review orders, and participate in education and quality initiatives to improve patient outcomes. The role emphasizes patient safety, pharmacist leadership, and opportunities to mentor interns and residents while supporting regional clinical programs and formulary

Qualifications

• Doctor of Pharmacy degree required with active license
• Minimum three years of Staff Pharmacist experience
• Missouri Pharmacist License required
• ACLS certification within 6 months of hire

Responsabilités

• Review and verify medication orders, ensuring safety and accuracy.
• Identify and resolve medication-related issues and drug interactions.
• Counsel patients and families on safe medication use.
• Lead clinical initiatives, quality improvement, and education programs.
• Collaborate with physicians, nurses, and pharmacists to optimize therapy.
• Document interventions in patient records and ensure formulary compliance.
