Job summary

ExcelaCare Home Health - Clovis, NM is seeking a Certified Occupational Therapy Assistant (PRN) to support patient rehabilitation in home settings. Under the supervision of an Occupational Therapist, you will implement treatment plans, educate patients and families, and help restore daily living activities.

The role requires completing planned therapies, documenting progress, and coordinating with the care team to ensure high-quality patient outcomes in a home health environment.

Qualifications

• Graduate of an accredited Occupational Therapy program for Certified Occupational Therapy Assistants; NBCOT eligibility or certification.
• 1 year home health experience preferred.
• CPR certification and valid driver’s license required.
• Ability to work with patients in home settings and follow physician plans of care.

Responsibilities

• Performs delegated therapies under supervision of an Occupational Therapist to meet patient goals.
• Instructs and assists patients with activities of daily living and therapeutic exercises.
• Implements home exercise programs and educates patients/families.
• Documents progress accurately and on time per policy.
• Participates in case conferences and interdisciplinary team meetings.
