Job summary

UT Health San Antonio is seeking a PRN Case Manager to provide as-needed care coordination in a hospital setting. You will assess patient needs, collaborate with the care team, and support discharge planning with families and post-acute services.

Expect variable shifts including on-call coverage. The role emphasizes patient-centered advocacy, documentation in EMR, and adherence to regulatory standards.

Qualifications

• Minimum of two years in case management, care coordination, social work, or nursing in a healthcare setting.
• Experience in acute care or hospital-based case management preferred.
• PRN/ per diem/ contract work arrangements are common in this role.

Responsibilities

• Conduct patient assessments to identify clinical, psychosocial, and discharge planning needs.
• Collaborate with physicians, nurses, and the interdisciplinary team to support coordinated patient care.
• Participate in care discussions and contribute to discharge planning activities.
• Identify and address barriers to patient throughput and timely discharge.
• Assist patients and families with discharge planning and post-acute services.
• Serve as liaison between patients, families, and the healthcare team.
• Advocate for patients’ needs to support safe care transitions.
• Provide education to patients and families regarding care plans and resources.
• Coordinate with community agencies to facilitate continuity of care.
• Document assessments and care plans in the EMR and maintain confidentiality.
