Job summary

Qualifications

• Three or more years of nursing experience in clinic or hospital settings.
• Experience in utilization management, case management, or discharge planning preferred.
• Experience coordinating care and advocating for patients.

Responsibilities

• Assist with comprehensive assessment, planning, implementation and evaluation of patient needs.
• Coordinate social services and case management functions with care and discharge processes.
• Act as patient advocate; monitor utilization and quality of care; report adverse events.
• Educate staff on resource utilization, discharge planning and psychosocial care.
• Communicate with physicians to align care plans and outcomes.
• Participate in policy development for patient care and discharge processes.
• Perform additional duties as assigned.
