Sussex Opco is seeking a PRN Caregiver in Wisconsin to provide compassionate, resident-centered care. You will assist with ADLs, meals, hydration, and social activities while promoting independence and dignity in a homelike environment.

Responsibilities include documenting care, following infection control policies, and reporting health changes promptly. Must be 18+, have a high school diploma, and be able to obtain a Wisconsin caregiver background check.

Responsabilidades

• Assist residents with ADLs such as bathing, dressing, grooming, toileting and mobility.
• Support residents with meals and hydration; encourage dining and activity participation.
• Provide companionship and engage residents in social activities.
• Perform light housekeeping and maintain residence areas.
• Monitor health changes and report promptly; administer medications if delegated and trained.

The PRN Caregiver is responsible for delivering compassionate, high-quality care to residents in accordance with their individualized service plans. This includes assistance with personal care, medication support (if delegated), household tasks, and activity engagement. The Caregiver plays a key role in fostering a safe, respectful, and homelike environment that promotes resident independence, dignity, and quality of life.Essential Duties & ResponsibilitiesAssist residents with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility.Support residents with meals, hydration, and dining assistance.Provide companionship and encourage participation in social and recreational activities.Perform light housekeeping tasks, including laundry, room organization, and upkeep of common living areas.Monitor residents' well-being and promptly report changes in condition or behavior.Administer medications and perform delegated health-related services, when authorized and trained under RN supervision.Maintain accurate documentation and resident care records.Follow infection control, confidentiality, and resident rights policies.Attend required training sessions and staff meetings.Comply with all local, state, and federal regulations.Required:At least 18 years of age.High school diploma or equivalent.Ability to pass a Wisconsin caregiver background check.Strong communication and interpersonal skills.Ability to work independently and as part of a team.Preferred:Previous caregiving, direct care, assisted living, memory care, or healthcare experience.CNA certification and/or CBRF training.Medication administration experience.Physical RequirementsAbility to stand, walk, bend, kneel, and assist residents throughout the shift.Ability to lift, push, pull, and transfer up to 50 pounds.Comfortable working in a residential healthcare environment, including exposure to cleaning products and bodily fluids.Reasonable accommodations may be made to enable qualified individuals with disabilities to perform essential job functions.
