Job summary

SSM Health is seeking an experienced Nurse Practitioner or Physician Assistant to join our Complex Care Clinic teams at DePaul Hospital Bridgeton and St. Joseph West Hospital Lake St. Louis.

This outpatient PRN role focuses on pre-surgical optimization and post-discharge care to improve outcomes. The position offers a flexible PRN schedule (minimum two shifts per month), outpatient-only work, solid onboarding, and a collaborative team culture with competitive hourly pay and malpractice coverage.

Qualifications

• Master's degree as a Nurse Practitioner or Physician Assistant.
• Minimum of 2 years of current experience in Internal Medicine or Adult Primary Care.
• Missouri NP licensure or Physician Assistant certification/licensure.
• Strong assessment, clinical judgment, and patient management skills.

Responsibilities

• Conduct pre-surgical evaluations and optimization using established clinical pathways.
• Collaborate closely with primary care providers and specialists to coordinate patient care.
• Evaluate patients following hospital discharge and provide timely assessment, treatment, or referrals as needed.
• Focus on clinical evaluation and care coordination with limited diagnostic workups and prescribing responsibilities.
