Responsibilities

The Registered Nurse in Home and Community-Based Services provides and directs nursing and personal care to patients in their homes, as prescribed by the physician or authorized by the state/program, in compliance with applicable laws, regulations, and agency policies. The RN coordinates the care plan with other healthcare professionals, ensuring continuity of care by planning and exchanging information with physicians, agency staff, patients, families, case managers, and community resources.

Education and Experience

Provides high-quality clinical services within the scope of practice and infection control standards, in accordance with the care plan and in coordination with the care team from admission through discharge.

Completes clinical nursing assessments as required by federal and state program requirements and payers.

Ensures patient eligibility and medical necessity for services per payer and agency policies.

Develops and revises individualized care and service plans according to federal and state requirements in collaboration with community providers.

Ensures the plan of care's frequency and duration meet patient needs, initiating revisions with physician approval as needed.

Education and Experience

Experience Desired: Minimum of one year as an RN; PEDS experience preferred.

License Requirements: Current RN licensure in practice state, valid driver’s license, vehicle insurance, access to reliable transportation, and current CPR certification.
