Job summary

Qualifications

• Current Arizona RN or LPN license.
• Current CPR/BLS certification.
• Ability to meet required background and credentialing requirements.

Responsibilities

• Provide one-on-one skilled nursing care in the patient's home.
• Follow the individualized plan of care.
• Administer medications and treatments as ordered.
• Monitor the patient's condition and communicate changes as needed.
• Complete accurate and timely documentation.
• Communicate with the family and clinical care team.
• Provide safe, compassionate, and dependable nursing care.
