Job summary

Health First is seeking an experienced registered nurse to provide skilled nursing care in home settings under a physician-prescribed plan. You will develop, implement, and revise care plans, coordinate interdisciplinary care, and manage patient safety and education.

Responsibilities include documenting care, communicating patient progress to the physician and Clinical Ops Supervisor, and supervising Home Health Aide staff when needed.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current Florida RN license.
• Current CPR from American Heart Association.
• ASHI BLS CPR card accepted; must obtain AHA BLS within 90 days.
• Successful completion of Level II background screening (fingerprinting).
• Current valid Driver License or ID card.

Responsibilities

• Coordinate delivery of nursing care across disciplines.
• Assess patient needs and implement a written plan of care.
• Document care and communicate progress to physician and supervisor.
• Supervise Home Health Aide as needed.
• Promote patient safety and learning during visits.
