Job summary

Interim HealthCare Inc. is seeking an RN or LPN to provide in-home direct patient care, following the prescribed plan of care and nursing scope. You will assess patient conditions, document changes, and administer medications and IV fluids, ensuring thorough documentation.

The role requires a current RN or LPN license in the relevant state, CPR/AED/BLS/First Aid certification, and reliable transportation to care sites. Ventilator experience is a plus.

Qualifications

• Graduate of a practical nursing program and has a current RN or LPN license in the relevant state(s).
• Current unrestricted license to practice as RN or LPN in the state of the position.
• Current CPR/AED/BLS/First Aid certification.
• Reliable transportation to/from care sites and/or work locations.
• Practical trach and/or ventilator experience preferred, not required.

Responsibilities

• Provide in-home direct patient care according to the prescribed plan of care and nursing scope of practice.
• Assess and document patient conditions and report changes as needed (vital signs, etc.).
• Administer medication, insulin, and IV fluids, documenting thoroughly.
• Inspect and care for wounds, change dressings, assist with personal hygiene.
• Assess patient for injuries, infections, bedsores, and deliver appropriate treatments.
• Collaborate with providers and other health professionals to ensure patient outcomes.
