Job summary

Avid Health at Home is seeking a Private Duty Nurse (RN) to join our client care team, delivering skilled nursing services to clients in their personal residences. The role reports to the Clinical Manager and requires travel within the service area.

The PDN-RN will ensure high-quality care, maintain documentation, and uphold HIPAA and agency policies while coordinating with families and health plan providers. This is a full-time, field-based nursing position.”

Qualifications

• Education: Graduate of an accredited school of nursing.
• Licensure/ Certification: Registered Nurse who is licensed by the NC Board of Nursing (NCBON). Must have a current, unencumbered RN license.
• Travel: Travel is required for business purposes; requires a valid driver’s license with satisfactory driving record.
• Work Experience: Minimum of one year of home care experience; an additional year in a hospital is preferred for training classes.
• Hours: Full time is at least 30 hours a week (Sat-Fri).
• Other Skills: Customer service oriented; able to liaise with patients, families and other care providers.
• Ability to use computerized systems; verbal and written communication; ability to work under pressure.

Responsibilities

• Evaluates and treats clients under supervision of an RN/Clinical Manager.
• Assist with continuity of care and emotional needs of client and family.
• Maintain accurate clinical and shift documentation in system per agency guidelines.
• Communicate client care information per agency policies to team and providers.
• Build relationships with clients to promote compliance with care plans and safety.
• Adhere to infection control and safety policies; participate in staff development and community outreach.
