Job summary

Legacy Concierge is seeking a dedicated Private Duty Registered Nurse to provide hands-on, 1:1 in-home care for clients in California. You will assess, monitor, and manage medical conditions, delivering skilled nursing tasks with supervision and mentorship within a clear clinical career path.

You will work independently with a focus on continuity of care, completing accurate documentation and collaborating with the RN Care Ambassador to ensure high-quality patient outcomes during each shift.

Qualifications

• Active California RN license in good standing.
• Graduate of an accredited ADN or BSN nursing program.
• Minimum hospital experience aligned to degree.
• BLS/CPR certification.
• Proven ability to work independently in a 1:1 home care setting.
• Strong communication and documentation skills.

Responsibilities

• Deliver 1:1 skilled nursing care in the patient's home.
• Perform comprehensive head-to-toe assessments including neurological, cardiovascular, and other systems.
• Monitor, document vital signs and changes in patient status.
• Administer medications and manage treatment plans as ordered.
• Support activities of daily living and instrumental tasks.
• Provide G/J-tube feeding and medication administration as ordered.
• Perform wound care and dressing changes; provide tracheostomy/ventilator care.
• Provide seizure monitoring and oxygen therapy as needed.
• Maintain HIPAA compliance and accurate documentation.
