Job summary

Legacy Concierge is seeking a dedicated Private Duty Registered Nurse to deliver exceptional 1:1 home care. Work independently with clinical supervision to assess, triage, and apply preventive care techniques while building trust with your patient during shifts.

Clinical onboarding will be completed; responsibilities align with each patient’s individualized plan of care, in a private home environment. Expect structured onboarding and ongoing competency evaluations.

Qualifications

• Active California RN license in good standing.
• Graduate of an accredited BSN nursing program.
• Minimum 5 years of hospital, in‑home concierge nursing, or skilled nursing experience.
• Current BLS/CPR certification.

Responsibilities

• Deliver 8–12 hours of 1:1 skilled nursing care in the patient’s home.
• Perform comprehensive head‑to‑toe assessments and monitor vitals.
• Manage medications safely, including insulin and other prescribed treatments.
• Support ADLs and IADLs such as toileting, dressing, feeding, and meal prep.
• Provide wound care and dressing changes; tracheostomy and ventilator care as needed.
• Monitor seizures and oxygen therapy; manage chronic conditions.
• Complete daily clinical notes and communicate changes with the RN Care Ambassador.
