Legacy Concierge is seeking a dedicated Private Duty Registered Nurse to provide 1:1 in-home care. You will perform hands-on clinical care, assessments, and medication administration, with mentorship and a clearly defined career path.

This role emphasizes patient-centered care, accurate documentation, and collaboration with the RN Care Ambassador. You will work independently in patients' homes, ensuring safety and continuity of care during each shift.

Qualifications

• Active California RN license in good standing.
• Graduated from an accredited ADN or BSN program.
• Current BLS/CPR certification.
• Experience in hospital, skilled nursing, or in-home care settings.
• Ability to work independently and document clearly.

Responsabilités

• Deliver 1:1 skilled nursing care in the patient's home.
• Perform head-to-toe assessments and monitor vital signs.
• Administer medications safely and document plan of care.
• Assist ADLs/IADLs and manage feeding or tubing as ordered.
• Provide wound care, tracheostomy/ventilator care as directed.
• Complete daily notes and communicate status to RN Care Ambassador.
