Job summary

Legacy Concierge is seeking a dedicated Private Duty Registered Nurse to provide high-quality 1:1 care in the patient’s home. You will work independently, applying clinical skills to assess, triage, and deliver hands-on care with mentorship and a clearly defined clinical career path.

Expect onboarding, patient-centered care, and competence development in a private home setting. You will complete plans of care, communicate with the care ambassador, and support family needs while upholding HIPAA

Qualifications

• Active California RN license in good standing (CA)
• Graduate of an accredited ADN or BSN nursing program
• Minimum hospital experience based on degree
• RNs with 5+ years hospital/ SNF/ in-home concierge experience encouraged
• Current BLS/CPR certification
• Proven ability to work 1:1 private care environment
• Strong communication, documentation and customer service skills

Responsibilities

• Deliver 1:1 skilled nursing care in the patient’s home
• Perform head-to-toe assessments and document findings
• Monitor and document vital signs during shifts
• Safely administer medications per plan of care
• Assist with ADLs/IADLs and meal prep
• Provide wound care and tracheostomy/ventilator care as directed
• Communicate changes with RN Care Ambassador and care team
• Maintain HIPAA compliance and patient confidentiality
