Job summary

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver’s license and vehicle insurance or access to transportation.
• Preferred CPR certification or ability to obtain within 90 days of hire.
• Ability to work flexible hours to meet client needs.
• Ability to manage multiple tasks and work independently.

Responsibilities

• Provides high quality clinical services within the scope of practice per the plan of care.
• Documents patient visits thoroughly per regulatory and payer requirements.
• Participates in care coordination, reporting needs and changes, and communicating with patient, family, physician and team.
• Function in any home situation regardless of client demographics.
• Contribute to development and growth opportunities within the role.
