Job summary

All Heart Home Care in Nashville, TN is seeking compassionate Licensed Practical Nurses (LPNs) for private duty home care, offering standard and floater roles across several Tennessee locations.

You will provide one-on-one nursing in clients’ homes, administer medications, monitor conditions, and educate families on care routines. We value flexible scheduling, reliable transportation, and a patient-focused approach.

Qualifications

• Active Licensed Practical Nurse (LPN) license in the state of employment.
• Previous private duty or home care experience preferred.
• Strong clinical and observational skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently and provide consistent, patient-focused care.
• Compassionate, reliable, and professional demeanor.
• Current CPR certification.

Responsibilities

• Provide direct, one-on-one nursing care in clients' homes.
• Administer medications and treatments as prescribed, within scope of practice.
• Monitor patient conditions and report changes to the care team.
• Assist with activities of daily living (ADLs) as needed.
• Perform routine nursing tasks such as wound care, vital signs, and basic treatments.
• Educate patients and families on care routines and health management.
• Maintain accurate and timely documentation of care provided.
• Ensure a safe, clean, and comfortable environment for clients.
• For Floater roles: adapt quickly to different home settings and patient needs.
