Job summary

Evolving Solution Services is seeking an experienced Homecare LPN to provide patient-centered nursing care in patients' homes across Michigan. You will assess conditions, administer medications, and perform wound and catheter care while coordinating with the care team to adjust plans as needed.

You will educate families, monitor progress, document findings, and ensure safety and adherence to nurse practice standards in a community setting, promoting independence and quality of life.

Qualifications

• Must hold a current and valid LPN license in the U.S.
• Completion of an accredited practical nursing program.
• BLS certification required.
• Minimum of one year of clinical nursing experience, preferably in home health or long-term care.
• Ability to pass background checks and health screenings.

Responsibilities

Minimum Qualifications:

• Current and valid Licensed Practical Nurse (LPN) license in the state of practice within the United States.
• Completion of an accredited practical nursing program.
• Basic Life Support (BLS) certification.
• Minimum of one year of clinical nursing experience, preferably in home health, long-term care, or related settings.
• Ability to pass background checks and meet health screening requirements.

Preferred Qualifications:

• Experience working in home health care or community-based nursing.
• Certification in wound care or chronic disease management.
• Familiarity with electronic visit verification (EVV) systems.
• Strong knowledge of infection control practices and patient safety protocols.
• Additional certifications such as IV therapy or geriatric nursing.
