Key Responsibilities

• Provide clinical services according to state laws, care plans, and infection control protocols.
• Thoroughly document patient visits in electronic medical records and visit verification systems.
• Participate in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, clinicians, and team members.
• Adapt to diverse home environments, respecting clients' backgrounds and conditions.

Qualifications & Skills

• Unrestricted LPN licensure in the practice state.
• Valid driver's license, reliable transportation or access to public transit.
• Preferred: CPR certification, flexibility, strong communication, organizational, and multitasking skills, and ability to work independently.
