Job summary

AccessHealth is seeking a Home and Community Based Services Licensed Practical Nurse to provide direct nursing care to acutely and chronically ill patients in their homes throughout Wisconsin. Under supervision, you will deliver high-quality clinical services, document visits thoroughly, and participate in care coordination to ensure optimal outcomes.

Responsibilities include following the care plan, documenting visits in EMR systems, and communicating with patients, families, and the healthcare

Qualifications

• Current, unrestricted LPN licensure in the practicing state.
• CPR certification and valid driver’s license with reliable transportation.
• Ability to work independently in diverse home environments.
