Job summary

Always Compassionate Health is seeking compassionate LPNs to provide one-on-one nursing care to patients in their homes in New York. The role offers flexible scheduling, immediate openings, and competitive pay, with opportunities to make a meaningful difference in patients’ lives.

Qualified applicants hold an active NY LPN license, CPR/BLS certification, and have at least 6 months of home care experience. Strong clinical and communication skills are essential, with the ability to lift up to 50

Qualifications

• Active New York State LPN license is required.
• Minimum of 6 months of home care experience.
• Current CPR/BLS certification.
• Strong clinical and communication skills.
• Compassionate, reliable, and able to work independently.
• Ability to meet physical requirements, including lifting up to 50 pounds.

Responsibilities

• Provide skilled nursing care and administer prescribed medications and treatments.
• Perform G-tube/PEG care, tracheostomy care, and suctioning as required.
• Monitor patient conditions and report changes promptly.
• Provide ventilator support when required and appropriately trained.
• Educate patients and caregivers and maintain accurate documentation.
