Responsibilities

• Perform nursing procedures within LPN scope of practice
• Educate patients and families per the care plan
• Monitor vital signs and observe patient conditions
• Record intake/output and report changes
• Assess and document physical and mental health status
• Administer medications as prescribed
• Participate in staff meetings and training

Qualifications

• Active LPN license in good standing
• CPR Certification
• Home health, private duty, or hospital experience preferred
• Strong communication and problem-solving skills
• Reliable transportation or valid driver’s license with insurance
