Responsibilities

1. Perform nursing procedures within LPN scope of practice
2. Provide patient and family education as outlined by RN
3. Monitor vital signs and record observations
4. Record intake and output, reporting significant changes
5. Assess and document patient's physical and mental condition
6. Administer medications as instructed by physicians and RNs
7. Participate in staff meetings and educational programs

Qualifications

• Active LPN license in good standing
• CPR certification
• Previous experience in home health, private duty, skilled nursing, or hospital preferred
• Strong communication and problem-solving skills
• Reliable transportation or valid driver’s license with insurance
