Responsibilities

1. Perform nursing procedures within LPN scope
2. Provide patient and family teaching as outlined by RN
3. Monitor vital signs and observations
4. Record intake/output and report changes
5. Assess and document physical and mental condition
6. Administer medications as instructed and document
7. Participate in case conferences, staff meetings, and training
8. Cooperate with on-call, weekends, and after-hours visits

Qualifications

* Valid LPN license in good standing

* Experience in home health, hospice, hospital, or skilled nursing preferred

* Familiarity with electronic health records

* Strong problem-solving skills

* Ability to work in a fast-paced environment

* Reliable transportation, valid driver’s license, and insurance
