Job summary

Tradition Homecare in Brooklyn, NY is seeking a dedicated Private Duty Homecare Coordinator to join our growing team. You will coordinate Private Duty Nurses (RNs/LPNs) and Home Health Aides (HHAs) to deliver seamless, high‑quality in‑home care for clients across the NYC area.

The ideal candidate has 3–5 years of hands‑on experience coordinating PDN and HHA services, exceptional organizational skills, and strong communication with clients, families, nurses, aides, and internal staff.

Qualifications

• 3–5 years of home care coordination or staffing experience required.
• Direct experience coordinating PDN and HHA cases required.
• Experience staffing RNs, LPNs, and HHAs.
• Familiarity with New York State home care requirements and compliance standards.

Responsibilities

• Coordinate and schedule Private Duty Nurses (RNs/LPNs) and Home Health Aides (HHAs) for client assignments in the NYC area.
• Manage staffing and coverage for Private Duty Nursing and HHA cases.
• Serve as primary point of contact for clients, families, nurses, HHAs and field staff.
• Address call-outs, scheduling changes, and urgent staffing issues.
• Assist with client intake, case setup, and coordination of individualized care needs.
