Job summary

Dartmouth Health is seeking a Spoke Practice Social Worker to deliver social work services to primary care patients within the Vermont Blueprint for Health Spoke Program. You will collaborate with the Community Health Team to address social determinants of health and coordinate resources.

The role requires strong knowledge of local, state, and federal programs, a valid social work license, and experience with adults, families, and children.

Qualifications

• Two years social work experience in a community, home health, or outpatient practice setting required.
• Knowledge of a variety of local, state, and federal resources required.
• Expertise in advanced care planning strongly preferred.
• Experience with interventions with adults, families, and children strongly preferred.
• Valid license in social work, vocational rehabilitation, substance abuse counseling, or related field required.
• Valid Driver's License and Auto Insurance required.

Responsibilities

• Provide social work services to primary care patients in the Spoke Program.
• Participate as an active member of the Community Health Team.
