Qualifications

• RN licensed in applicable state with current license.

• Excellent communication and teamwork skills.

Responsibilities

• Post-hospital follow-up calls for patients at risk of readmission.
• Home visits to patients who qualify for adult medical care coordination.
• Collaborative care with primary care providers to develop care plans.
• Panel management with continued follow-up and progress monitoring.
