Qualifications

• Active RN license in Alaska (board-certified).
• 2-4 years of nursing experience (primary or chronic care).
• Experience with case management.
• Chronic care & diabetes support.
• Injections & phlebotomy.
• Triage & inbox management.
• Prior authorizations.
• Strong assessment, care planning, and patient education.
• EMR experience and strong documentation skills.
• BLS certification required.

Responsibilities

• Assess patient needs and document findings in the EMR.
• Provide case management support for chronic care and diabetes.
• Administer medications, immunizations, and treatments within RN scope.
• Perform injections and phlebotomy as needed.
• Support providers during exams, procedures, and diagnostic testing.
• Manage triage calls, inbox messages, and patient follow-ups.
• Complete prior authorizations and coordinate care plans.
• Monitor patient status and escalate changes appropriately.
• Educate patients and families on care plans, medications, and preventive care.
• Maintain compliance with clinical policies, infection control, and documentation standards.
