Vera Whole Health in Anchorage, AK is hiring a Registered Nurse (RN) for Primary Care & Case Management. The role blends clinical tasks with back‑office care coordination, chronic disease support, and patient education.

You’ll triage, manage inbox messages, and support providers while maintaining EMR documentation and quality standards. Monday–Friday schedule with competitive benefits awaits.

Qualifications

• Active RN license in Alaska required.
• 2–4 years of nursing experience in primary/chronic/diabetes care preferred.
• Experience with case management, injections, phlebotomy, triage, and inbox management.
• Strong assessment, care planning, and patient-education skills.
• Solid EMR documentation and BLS certification.

Responsabilités

• Assess patient needs and document findings in the EMR.
• Provide case management for chronic and diabetes care.
• Administer medications, injections, and treatments within RN scope.
• Support providers during exams, procedures, and testing.
• Manage triage calls, inbox messages, and patient follow-ups.
• Complete prior authorizations and coordinate care plans.
• Educate patients and families on plans, meds, and preventive care.
• Maintain policy, infection control, and documentation standards.
• Collaborate with interdisciplinary team for whole-person care.
