Job summary

Baystate Health is seeking an experienced Primary Care Nurse for the Northern Edge Adult & Primary Care Practice in Springfield. This full-time role supports triage, patient education, and plan of care for complex patients, including those with substance use health conditions.

Requirements include MA RN licensure, BLS CPR, and at least 2 years of outpatient nursing experience. Sign-on bonus available for external hires.

Qualifications

• Massachusetts RN license and BLS CPR certification required.
• Minimum 2 years of nursing experience in outpatient or primary care.
• Experience caring for patients with substance use diagnoses or related conditions.
• Ability to work with diverse populations and collaborate with physicians.

Responsibilities

• Provide triage and patient care with strong assessment skills.
• Educate and support patients with chronic substance use health conditions.
• Develop and implement care plans for complex patients with medical and psychosocial needs.
• Coordinate access to care and advocate for patients' rights.
