Job summary

Baystate Health is seeking an experienced Primary Care Nurse to join the South Hadley Adult Medicine Practice. This full-time role offers a 40-hour workweek with no weekends or holidays and a sign-on bonus eligible for external hires.

Responsibilities include triage, patient care, and coordination with the medical team to develop comprehensive care plans for complex patients. A Massachusetts RN license with BLS CPR is required, along with at least 2 years of nursing experience.

Qualifications

• Massachusetts license as an RN and BLS CPR certified.
• Minimum of 2 years of nursing experience.
• Experience providing clinical care, education, and treatment to patients with substance abuse or primary care.
• Demonstrated interest to work with patients from diverse backgrounds.
• Excellent verbal and written communication and interpersonal skills.
• Competency in delivering exceptional nursing care and collaborating with physicians and staff.

• Full Time Monday - Friday, No weekends or holiday
• 4- 10 hours days 7AM- 5:30PM - Thursdays off

Responsibilities

• Provides independent quality triage and patient care utilizing strong assessment skills, point of care testing and quality assurance checks
• Provides education, ongoing assessmentand support to patients with chronic substance use health conditions
• In partnership with patient and medical staff, develop and implement plan of care for complex patients including medical and/or psychosocial disorders.
• Triages and coordinates patient care ensuring access to care and patient success.
• Promotes health and wellness for individuals and groups of patients.
• Demonstrates respect and advocates for the patient’s rights, values and belief systems.

Required Work Experience:

• Massachusetts license as an RN and BLS CPR certified.
• Minimum of 2years of nursing experience
• Experience providing clinical care,education, and treatment to patients with substance abusediagnosesor primary care experience
• Demonstrated interest to work with patients from diverse economic and cultural backgrounds
• Excellent verbal and written communication and interpersonal skills.
• Must have competency in delivering exceptional nursing care and have proven success in partnering with both physicians and staff

• Location -470 Granby Road
• Full Time Monday - Friday No weekends or holiday
• 4- 10 hours days 7AM- 5:30PM - Thursdays off
• Sign On Bonus eligible- Must have 1 year of outpatient experience

No employee will be paid below the minimum of the range. Pay ranges are listed as hourly for non-exempt employees and based on assumed full time commitment for exempt employees.

$72,800.00 - $98,800.00 - $130,728.00
