Qualifications

• Completion of an accredited residency in Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics.
• Unrestricted state physician license, CDS, and DEA registration are required.
• Board certification preferred; CPR and First Aid certification required.

Responsibilities

• Conduct thorough in-person assessments and provide high-touch primary care.
• Lead Interdisciplinary Team (IDT) meetings to craft holistic care plans.
• Oversee transitions of care, including acute episodes and long-term placement management.
• Advocate for patients within Medicare/Medi-Cal systems.
