Job summary

Staffing-Agency-Recruitment is seeking a Primary Care Physician to join a mission-driven PACE program in California, delivering comprehensive, value-based care to vulnerable seniors. Expect about 8 patient visits per day, with no hospital rounds and plenty of meaningful patient relationships.

Schedule is Monday–Friday, 8:00 AM–5:00 PM, with home visits and rotating on-call. Benefits include relocation assistance, medical/dental/vision, 401(k) match, PTO, sick days, holidays, CME reimbursement,

Qualifications

  • Board-certified physician with MD or DO.
  • Active, unrestricted state medical license.
  • DEA and CDS certifications (state) in good standing.

Are you a Primary Care Physician who wants to spend more meaningful time with patients instead of managing an overwhelming patient volume or making hospital rounds?

This is an opportunity to provide comprehensive, community-based care to vulnerable seniors through a mission-driven PACE program.

You’ll work with a multidisciplinary team to deliver personalized, whole-person care while building meaningful long-term relationships with your patients.

With an average of approximately 8 patients per day and no hospital rounds, this role gives you the opportunity to focus on quality rather than simply volume.

Your Schedule

You’ll generally work Monday-Friday, 8:00 AM-5:00 PM. The position includes home visits and a rotating phone-only on-call schedule, typically involving a few nights or weekends per month.

There are no hospital rounds, allowing you to concentrate on comprehensive primary care.

What You’ll Do
  • Conduct comprehensive in-person assessments of PACE participants.
  • Develop personalized care plans in partnership with the interdisciplinary team (IDT).
  • Collaborate with nurses, social workers and other healthcare professionals.
  • Coordinate acute, short-term and long-term care placements and discharges.
  • Provide comprehensive primary care for senior patients.
  • Participate in home visits.
  • Help coordinate the patient’s overall medical care and long-term needs.
  • Work collaboratively to improve patient outcomes and quality of life.
Who We’re Looking For

We’re seeking physicians with backgrounds in:

  • Internal Medicine
  • Family Medicine
  • Geriatrics
  • Emergency Medicine

The ideal candidate has 2-5 years of primary care experience, preferably involving geriatric or senior populations.

Experience working within an Interdisciplinary Team (IDT) is highly relevant.

Experience leading or supervising Nurse Practitioners or Physician Assistants is also valuable.

Required Qualifications
  • Successful completion of an ACGME-accredited residency program
  • Master’s or Doctorate degree
  • Active, unrestricted state physician license
  • State CDS certification
  • DEA registration
  • Eligibility to enroll as a provider serving Medicare and Medi-Cal participants
  • CPR and First Aid certification, or ability to obtain certification within 180 days of hire

Board Certification Is Preferred.
