Are you a Primary Care Physician who wants to spend more meaningful time with patients instead of managing an overwhelming patient volume or making hospital rounds?
This is an opportunity to provide comprehensive, community-based care to vulnerable seniors through a mission-driven PACE program.
You’ll work with a multidisciplinary team to deliver personalized, whole-person care while building meaningful long-term relationships with your patients.
With an average of approximately 8 patients per day and no hospital rounds, this role gives you the opportunity to focus on quality rather than simply volume.
You’ll generally work Monday-Friday, 8:00 AM-5:00 PM. The position includes home visits and a rotating phone-only on-call schedule, typically involving a few nights or weekends per month.
There are no hospital rounds, allowing you to concentrate on comprehensive primary care.
We’re seeking physicians with backgrounds in:
The ideal candidate has 2-5 years of primary care experience, preferably involving geriatric or senior populations.
Experience working within an Interdisciplinary Team (IDT) is highly relevant.
Experience leading or supervising Nurse Practitioners or Physician Assistants is also valuable.
Board Certification Is Preferred.

Skills: geriatric medicine - experience caring for elderly/senior patients, ideally in a pace or similar value-based setting., physician licensing & dea/cds compliance - active unrestricted state medical license plus dea registration and state cds certification., interdisciplinary team (idt) collaboration - ability to work with nurses, social workers, therapists, and other healthcare professionals to create care plans., primary care / internal medicine - strong experience providing comprehensive primary care., patient assessment & care planning - performing comprehensive assessments and developing individualized treatment/care plans.
