Heartline Staffing is recruiting a Primary Care Physician to provide value-based healthcare to Medicare patient populations. The physician will work on-site at a single medical center and be supported by a large care team to focus on quality of care and patient outcomes within a value-based care model.

Responsibilities include assessing and diagnosing patients, directing primary care services, prescribing medications, and maintaining EMR data using Canopy and Greenway.

Heartline Staffing is recruiting a Primary Care Physician to provide value‑based healthcare to Medicare patient populations. Physicians practice at a single medical center supported by large care teams, allowing providers to focus on quality of care rather than volume of services. The value-based care model is structured around health outcomes, preventive medicine, low hospital admissions, and patient experience.

COMPENSATION

• $174,070-$374,920 base annual salary range
• Actual base salary offer depends on experience, education, geography, and other relevant factors

Requirements

• Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) graduate REQUIRED
• Internal Medicine or Family Medicine Board Certified or Board Eligible REQUIRED
• Active, non-probationary, unrestricted state license REQUIRED
• Active DEA license REQUIRED
• U.S. work authorization REQUIRED

Preferred

• Fellowship training in Geriatrics
• Experience practicing medicine among geriatric populations
• Experience operating in a Value-Based Healthcare Model
• Experience working in a collaborative setting to ensure positive health outcomes
• Experience in outpatient primary care settings
• Bilingual proficiency where applicable
