Job summary

BoldAge PACE is seeking a Provider Services NP to deliver primary care and continuous healthcare services to our participants in Oceanport, NJ. You will assess, plan, and provide care in alignment with PACE standards and work closely with the clinical care team to develop care plans.

This full-time role requires MD/DO/APRN/PA credentials, NJ licensure, and board certification in geriatrics preferred. You will participate in daily IDT meetings, manage medications, and ensure accurate

Qualifications

• MD, DO, APRN, or PA credential.
• NJ State board certification/licensure to practice in New Jersey.
• State CDS Certification and DEA Registration.
• Board Certified in Geriatrics preferred.
• BLS required (within 90 days of employment).
• 2 years direct patient care experience preferred.
• 1 year experience with frail or elderly population preferred; training provided if needed.

Responsibilities

• Provide high-quality primary care with individualized evaluation and a comprehensive history and exam.
• Collaborate with IDT before referring participants to specialists to ensure necessity and continuity.
• Manage and monitor participants' medications in coordination with Medical Director and pharmacist.
• Participate in daily IDT meetings and contribute to participant-centered care plans.
• Document assessments, orders, and plans accurately for policy and quality reporting.
