Job summary

BoldAge PACE in Oceanport, NJ seeks a Provider Services NP/PA/MD to deliver primary care to PACE participants and work with the clinical team to assess, plan, and execute care per PACE standards. You will perform assessments, develop care plans, and support comprehensive primary care.

This full-time role requires NJ licensure, geriatrics certification preferred, and 2+ years of direct patient care. You will manage medications, coordinate care, and participate in on-call rotations to ensure

Qualifications

• MD/DO/APRN/PA required or eligible.
• New Jersey licensure required for practice.
• Board certification in geriatrics preferred.
• BLS certification required within 90 days of employment.
• Minimum 2 years of direct patient care experience.
• Experience with frail or elderly populations preferred.

Responsibilities

• Deliver high-quality primary care with individualized evaluation and comprehensive history and exam.
• Collaborate with IDT before referrals to specialists to ensure necessity and continuity of care.
• Coordinate with Medical Director on referrals, treatment, and communication of care plans.
• Manage participants' medications in coordination with Medical Director and pharmacist.
• Monitor patient status during hospitalizations and facility stays for safe transitions.
• Contribute to discharge planning and hand-offs across care settings.
• Participate in daily IDT meetings and quality initiatives.
• Ensure compliance with policy, regulatory, and quality standards.
• Educate participants and caregivers on self-care and wellness practices.

Education
