Job summary

CenterWell Senior Primary Care in Shreveport, LA is seeking a dedicated Primary Care Physician for a PRN (as-needed) role providing outpatient care to seniors. You will work onsite across Shreveport and Bossier City, collaborating with a multidisciplinary team to deliver high-quality care with emphasis on continuity and prevention.

The role requires board-certified MD/DO, active state license, and DEA/MediCal numbers or the ability to obtain them. TB screening may be required.

Qualifications

• MD or DO degree with board certification.
• Active, unrestricted license in the state of practice.
• Active DEA license and Medicare/Medicaid numbers or ability to obtain DEA license and Medicaid/Medicare numbers.
• TB screening may be required.

Join a Team That's Redefining Senior Primary Care

We are looking for a dedicated and experienced Primary Care Physician to join our team on a PRN (as-needed) basis. This is a zero-hour contracted role, ideal for providers looking for flexible scheduling while contributing to high-quality outpatient care. You'll manage a variety of health-related concerns, focusing on continuity of care, health maintenance, and disease prevention.

You will work onsite at our clinics across Shreveport and Bossier City reporting to our Associate Medical Director.

Your Role & Responsibilities

• You will provide comprehensive care to seniors following clinical standards.
• You will collaborate daily with your care team to ensure coordinated, high-quality care.
• You will maintain accurate and up-to-date medical histories and records.

What We're Looking For

• You will have a MD or DO degree with board certification.
• You will hold an active, unrestricted license in the state of practice.
• Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and Medicaid and Medicare numbers
• If selected for this role, we will require you to screen for TB.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$310,000 - $368,500 per year
