Primary Care Physician (PCP) - Summary

Duties & Responsibilities

• Provide comprehensive primary care to seniors with multiple chronic conditions in a full-time, outpatient setting
• Manage a patient panel of 18–20 patients per day, focusing on quality and outcomes
• Deliver value-based care that emphasizes patient relationships and long-term health improvement
• Collaborate with a dedicated support team, including a Medical Assistant, scheduler, and referral specialist
• Develop and implement individualized care plans for patients with 3–5+ chronic conditions
• Coordinate care with specialists and other providers as needed
• Participate in continuous quality improvement initiatives
• Utilize electronic medical records (EMR) to document patient encounters and track outcomes

Salary & Benefits

• Competitive base salary starting at $250,000+ per year
• Incentive bonus up to 10% of base salary
• Signing bonus up to $50,000
