Job summary

Avanta Health is seeking a dedicated Primary Care Physician to join our Fountain Valley, California practice. The physician will provide comprehensive adult primary care with emphasis on preventive care and chronic disease management.

The candidate should be board certified/eligible in Family or Internal Medicine and hold an unrestricted California medical license. New graduates are encouraged to apply and may receive signing incentives based on experience.

Qualifications

• MD or DO degree from an accredited institution.
• Residency in Family Medicine, General Practice, or Internal Medicine.
• Board Certified or Board Eligible in Family Medicine or Internal Medicine.
• Current, unrestricted California physician license.
• Current DEA registration.
• Current BLS and ACLS certifications.
• New graduates completing residency are highly encouraged to apply.

Responsibilities

• Provide comprehensive primary care, including examinations, diagnosis, treatment, and follow-up.
• Manage acute and chronic conditions, including diabetes, hypertension, COPD, cardiovascular disease, and other common conditions.
• Provide preventive care, annual wellness visits, health screenings, and patient education.
• Order, review, and follow up on laboratory tests, imaging, and other diagnostic studies.
• Prescribe medications and monitor treatment effectiveness.
• Maintain accurate and timely electronic health records and clinical documentation.
• Coordinate care with specialists, nurses, medical assistants, and other healthcare professionals.
• Participate in quality improvement, care management, and population health initiatives.
• Promote continuity of care and a positive patient experience.
• Perform other related duties and responsibilities as assigned based on the needs of the practice.
