Job summary

A culturally-focused healthcare organization seeks a Primary Care Physician to deliver holistic care within a PACE program in Alhambra, CA. Qualifications

• Active board certification in California for relevant clinical areas.
• Willingness to obtain medical license in California if not currently licensed.
• Must be currently enrolled or eligible for enrollment to provide services to Medicare and Medi-Cal.

Responsibilities

• Provide comprehensive primary care to participants.
• Develop individualized care plans with the interdisciplinary team.
• Coordinate with community-based physicians for discharge planning.
• Participate in daily huddles and interdisciplinary meetings.
