Qualifications

• Evaluate and treat patients with acute and chronic conditions using evidence-based guidelines.

Responsibilities

• Perform history, physical exams, and diagnostic interpretation to guide treatment.
• Develop individualized treatment plans including medication management and preventive services.
• Order and interpret labs, imaging, and ECGs to establish diagnoses.
• Lead office-based procedures aligned with credentialing.
• Conduct annual wellness visits and preventive screenings.
• Coordinate care across specialists and care teams.
