Job summary

Community LIFE in Pittsburgh, PA is seeking a Primary Care Physician to oversee geriatric care in our on-site clinic. Expect 4–6 patient visits daily, with a team-focused approach to planning and delivering comprehensive services to frail seniors.

Requires MD/DO, residency in Internal or Family Medicine, PA license, and DEA; geriatrics fellowship is preferred. Relocation assistance may be available and shift is Mon-Fri with limited on-call.

Qualifications

• Graduate of an accredited medical school with residency in Internal or Family Medicine.
• Fellowship in geriatrics is preferred for this role.
• Must hold PA medical licensure and DEA with Medicare enrollment.

Responsibilities

• Perform periodic and episodic assessment and management of medical conditions for participants.
• Oversee referrals to medical specialists and inpatient care as needed.
• See 4–6 patients daily in the on-site clinic; participate in the interdisciplinary team.

Shift: Mon-Fri, 7:30am-4:00pm plus limited on-call

Location: Pittsburgh, PA - Relocation assistance may be available

Summary of Job Duties: Our Primary Care Physician is responsible for periodic and episodic assessment and management of individual participants’ medical conditions, and for oversight of their use of medical specialists and inpatient care. Our Physician typically sees 4-6 patients per day. Patients are seen in our on-site clinic, plus occasional visits to homes or SNFs. The Physician is part of an Interdisciplinary Team who works together to assess, plan care for, and provide service to frail, elderly participants.

Required Education: Graduate of an accredited school of medicine. Completion of residency in Internal Medicine or Family Medicine. Fellowship training in geriatrics preferred.

Required Licensure:Must have a current license to practice medicine in Pennsylvania (MD or DO), current DEA license, Medicare provider enrollment, and a valid PA driver's license.

Required Experience:A minimum of one year of primary care experience with a frail or elderly population.

Required Skills:

• Geriatric Assessment and management of co-morbidity.
• Effective listening and communication skills with patients, families and IDT members.
• Clear and complete documentation.
• Effective Team skills including but not limited to reliability, active listening, positive approaches, courtesy and respect in line with Community LIFE Mission and Values statements.
