Job summary

Northeast Medical Group within Yale New Haven Health is seeking Board-Certified Family Medicine or Internal Medicine physicians to join a collaborative team with an APRN. Qualifications

• MD or DO with ABIM-recognized Internal Medicine or Family Medicine residency (three years minimum).
• Professional clinical experience in Internal Medicine or Family Medicine preferred.
• Valid American Board certification in Internal Medicine or Family Medicine, and Connecticut license.
• Thorough knowledge of pharmacological agents and ability to lead medical staff.

Responsibilities

• Evaluate patients and treat acute and chronic illnesses.
• Order, perform and interpret diagnostic tests.
• Involve patients and families in care decisions.
• Create treatment plans and complete orders in charts.
• Collaborate with staff, physicians, and management to improve care delivery.
• Initiate appropriate specialty referrals.
• Provide clinical information to the team in a timely manner.
• Ensure accurate and timely patient documentation.
• Work with management on billing, coding and CPT code assignment.
• Aim for optimal patient access and interdepartmental coordination.
