Northeast Medical Group, part of Yale New Haven Health, seeks an Internal/Family Medicine physician to join a collaborative team of three physicians and an APRN. You will provide comprehensive care, manage chronic diseases, and offer same-day access in a community-focused setting.

Formación

• Degree of MD or DO with ABIM-recognized Internal Medicine or Family Medicine residency (three years minimum).
• Valid American Board certification in Internal Medicine or Family Medicine.

Responsabilidades

• Evaluate patients, and treat acute and chronic illnesses
• Orders, performs and interprets diagnostic tests
• Involves patients and their families in care and treatment decisions
• Creates treatment plans and completes orders in patient's charts
• Collaborates on improvements in care delivery and service with staff, other physicians and management
• Initiates appropriate specialty referrals
• Provides clinical information to the interdisciplinary team in a clear and timely manner
• Ensures that proper patient documentation is timely, accurate and complete
• Works with practice management to ensure proper compliance with billing, coding and CPT code assignment
• Achieves and maintains optimal patient access
• Provides interdepartmental medical consultation as requested
• Participates in the development of medical protocols, standard procedures and management of
