Job summary

Yale New Haven Health is seeking a board-certified MD or DO physician to join our Northeast Medical Group in Connecticut. You will evaluate patients, manage acute and chronic illnesses, and lead care planning with interdisciplinary teams across multiple hospitals.

Qualifications

• MD or DO with ABIM- or ABFM-recognized residency (three years minimum).
• Valid board certification in internal medicine or family medicine.
• Connecticut medical license or active license in process and ready to transfer.

Responsibilities

• Evaluate patients, and treat acute and chronic illnesses.
• Orders, performs and interprets diagnostic tests.
• Involves patients and families in care decisions.
• Creates treatment plans and completes orders in charts.
• Collaborates with staff, physicians, and management to improve care delivery.
• Initiates appropriate specialty referrals.
