Job summary

Northeast Medical Group, part of Yale New Haven Health, seeks a Primary Care physician for an outpatient practice in New Britain, CT. The role supports startup expansion and collaboration with leadership to design/refine clinical workflows, patient pathways, and scheduling models.

Responsibilities include patient evaluation, diagnostic testing, care coordination, and interdepartmental collaboration to improve care delivery and access. Relocation assistance and a signing bonus may be offered.

Qualifications

• Degree of MD or DO with ABIM-recognized internal medicine residency (three years minimum) or ABFM-recognized family medicine residency (three years minimum).

Responsibilities

• Evaluate patients, and treat acute and chronic illnesses.
• Orders, performs and interprets diagnostic tests.
• Involves patients and their families in care and treatment decisions.
• Creates treatment plans and completes orders in patient's charts.
• Collaborates on improvements in care delivery and service with staff, other physicians and management.
• Initiates appropriate specialty referrals.
• Provides clinical information to the interdisciplinary team in a clear and timely manner.
• Ensures that proper patient documentation is timely, accurate and complete.
• Works with practice management to ensure proper compliance with billing, coding and CPT code assignment.
• Achieves and maintain optimal patient access.
• Provides interdepartmental medical consultation as requested.
• Participates in the development of medical protocols, standard procedures and management.

Education
