Qualifications

• Unrestricted medical licensure in Indiana or ability to obtain it prior to start.
• Board certification or eligibility in Family Medicine, Internal Medicine or Med-Peds.
• Active DEA License or ability to obtain it before start.

Responsibilities

• Provide comprehensive primary care to a diverse patient panel.
• Collaborate with a multidisciplinary team.
• Perform in-office procedures if desired.
• Participate in quality improvement initiatives.
• Educate patients about their health.
