Qualifications

• Unrestricted CT medical license or eligibility to obtain before start.
• Board certification/eligibility in Family Medicine or Internal Medicine.
• Active, unrestricted CSR and DEA, or eligibility to obtain before start.

Education

Unrestricted Connecticut medical license (or eligible)

Board-certified/board-eligible in Family Medicine or Internal Medicine

CSR and DEA license (or eligible)
