Job summary

The Lakeland VA Healthcare Clinic is seeking a physician to provide comprehensive primary care under the Patient Aligned Care Team (PACT) model. The physician will deliver preventive and chronic care to an established panel, in a VA system environment.

The role requires an MD or equivalent, full US licensure, and board eligibility in a core specialty. Participation in ED RP and Faculty status considerations may apply.

Qualifications

• Must be a Doctor of Medicine or equivalent from VA-approved schools.
• Must hold a full and unrestricted license to practice medicine in the US.
• Residency in an accredited core specialty leading to board eligibility.
• Board certification for faculty status may be required.
• Non-US residency with minimum post-training practice in the US may be acceptable.

Responsibilities

• Provide comprehensive primary and preventive care to an established patient panel within the PACT model.
• Manage adult patients with acute and chronic conditions following VA clinical guidelines.
• Collaborate with the health care team and document care in the EMR.
• Deliver evidence-based medicine and patient education.
• Participate in professional development and performance improvement activities.
