Operating as a payor-agnostic, wholly owned subsidiary of Humana, our centers prioritize the unique needs of seniors. Our Clinics offer a team-based care model where physicians lead a multidisciplinary team supporting patients' physical, emotional, and social wellness.

The Primary Care Physician (PCP) works as a lead in our team-based care environment. We are a value-based care provider focused on quality of care. Our care team includes doctors, advanced practice professionals, pharmacists, care coaches, nurses, medical assistants, behavioral health specialists, and more. Our model aims to provide higher quality care and better outcomes for seniors through a concierge experience, multidisciplinary services, coordinated care supported by analytics, and strong community ties. This support enables our PCPs to see fewer patients and dedicate more time to each.

Responsibilities:
  1. Evaluates and treats patients in accordance with care standards.
  2. Monitors the quality of care using data and chart reviews.
  3. Coordinates patient services, including referrals and hospital/SNF coordination.
  4. Participates actively in daily team huddles as a medical expert.
  5. Supports a cooperative practice environment with professionalism and approachability.
  6. Completes medical records promptly and works with coders to optimize coding accuracy.
  7. Follows policies and protocols set by Clinical Leadership.
  8. Reviews care quality and outcomes with the Regional Medical Director.
  9. Contributes to the growth of services within the center.
  10. Participates in the local primary care on-call program as needed.
  11. Ensures compliance with licensing, certification, and accreditation standards.
  12. Focuses 100% of clinical time on direct patient care, including documentation and meetings related to patient care.

Required Qualifications:
  1. Current and unrestricted medical license or willingness to obtain one in the practice state; eligible for licenses in other states as needed.
  2. Graduate of an accredited MD or DO program.
  3. Excellent communication skills.
  4. Strong interpersonal skills with colleagues and patients.
  5. Committed to team-based care models.
  6. Willing to learn and adapt to a value-based care setting.
  7. Superior patient/customer service skills.
  8. Basic computer skills, including email and EMR.
  9. This role involves TB screening; candidates will be screened for TB.
Preferred Qualifications:
  1. Board certification or eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.
  2. Active and unrestricted DEA license.
  3. Medicare Provider Number.
  4. 2-5 years of relevant experience.
  5. Experience managing Medicare Advantage panels.
  6. Knowledge of Medicare guidelines and HEDIS indicators.

Pay Range

The estimated annual pay range for full-time employment is $203,400 - $299,500, depending on location and experience. The role is eligible for bonus incentives based on performance.
